I/We authorize UnionBank of the Philippines or its authorized representative to, pursuant to BSP Circular No. 472 Series of 2005 as implemented by BIR Revenue Regulation RR-4- 2005, conduct random verification with the Bureau of Internal Revenue in order to establish authenticity of the Income Tax Returns (ITR), accompanying financial statements and such other documents/information/data submitted by me/us.
3. I/We authorize UnionBank of the Philippines or any of its authorized representatives to share my personal data with Data Analytics Ventures, Inc. to facilitate enrollment and points crediting in the Go Rewards Program in the event that my/our application is approved and I/we fully understand that said information may be used by the Bank to provide the service and/or product I have applied for from UnionBank of the Philippines. In case of any inquiries and concerns on the processing of my personal information, I/we may contact the Data Protection Officer through dpo@unionbankph.com.
4. I/We hereby agree to be bound by the Terms and Conditions of debit card application promos (whenever applicable) and the Terms and Conditions governing the issuance and use of the Go Rewards Visa Debit Card and Savings Account.
